GBNHC - Waiver & Release

RELEASE & HOLD HARMLESS AGREEMENT: Please read this form carefully and be aware in registering your minor child for participation in one or more Glenbrook North Hockey Club (“GBNHC”) hockey programs, you will be waiving and releasing all claims for injuries your player might sustain arising out of such program(s). I recognize and acknowledge that there are certain risks of physical injury to participants in hockey and I agree to assume voluntarily the full and entire risk of any injuries, damages or loss, regardless of severity, which I or my player may sustain as a result of participating in any or all activities connected or associated with such programs. I agree to waive and relinquish all claims I or my player may have (or accrue to me or my player) as a result of participating in the program(s) against the GBNHC and any of its Directors, Coaches, Board Members, officers, members, volunteers, agents or employees. I further agree to indemnify and hold harmless and defend The Northbrook Park District (“District”) and any of its Board Members, officials, officers, affiliates, volunteers, agents or employees from any and all claims resulting from injuries, damages or losses sustained by me or my player arising out of, connected with, or in any way associated with the activities of the GBNHC program(s). In the event of any emergency, I authorize the GBNHC to secure from any licensed hospital, physician or medical personnel and any treatment deemed necessary for me or my player's immediate care and agree that I will be responsible for payment of all medical services rendered. I have read and fully understand the above Waiver and Release of All Claims and Permission to Secure Treatment.
